Before You Begin
- The People Information Submission forms give LabraData permission to enter you and your dogs into the database. Dogs or Litters can not be linked to owners or breeders with out proper permission to display the individual in the database.
- Complete a Personal Information form for yourself prior to submitting any other information. You must be in the database to be linked to any dog record as either an owner or breeder.
- If you co-own a dog or co-bred a litter, the co-owner(s) must also submit a Personal Information form in order to be listed.
- For Dog Data Submission: Complete a separate form for each dog. A copy of the registration may be uploaded directly on the submittal form or mailed to filemanager@labradata.org (see Scanned Documents or Mailed Documents for details). A dog can not be entered without this information unless it is unregistered.
- For Litter Data Submission: Complete one form for the entire litter. If the litter has more than 12 puppies, submit another form and be sure to note that it is ADDITIONAL information for a litter in the comments section.
Dogs will be linked to a breeder through this form, but not to an owner. The owner must submit a People Information form and Dog Data form to "claim" the dog even if it is in the database as the result of a litter submission.
- Please note that all awards, tests, and certifications require verification. If scanned documents are available, they may be uploaded once the dog has been entered into the database and will be viewable for each entry. If scanned documents are not available, any award, test or certification that is verifiable from public records may be included on the Dog Data Submission form. They will be checked against available sources when they are entered.
- Health diagnoses may be anecdotal and self-reported by the owner. For a diagnosis to be considered "verified" the report requires a supporting document to be submitted (see Scanned Documents or Mailed Documents).
